Subject: REQUEST FOR APPROVAL<\/p>

Step 2: Attach the following:<\/p>

  • HMO Card or Certificate of Coverage<\/li>
  • Valid ID (plus guardian\u2019s ID if for a minor)<\/li>
  • Doctor\u2019s request with diagnosis<\/li>
  • For procedures with an appointment, include the date<\/li><\/ul>

    Step 3: Include a brief medical history explaining your reason for consultation.<\/p>

    When to send your request?<\/p>

    Send it at least 1 day before your scheduled visit or procedure.<\/p>

    Avoid delays or denial:<\/p>

    Make sure all required files are complete. Missing info may cause delays or denial of approval.<\/p>

    On the day of your appointment:<\/p>

    Bring your physical HMO card or valid ID when claiming your LOA.<\/p>

    Note:<\/p>
