1. Right to Appropriate<\/strong> Medical Care Humane Treatment<\/strong>
(Karapatang gamutin ng maayos at mabigyan ng sapat at makataon pag-aalaga anuman ang katayuan sa buhay.)

2. Right to Self-determination<\/strong>
(Karapatang makapag desisyon sa sarili para sa pangangalaga.)

3. Right to Informed Consent<\/strong>
(Karapatang mabigyan ng malinaw at makatotohanang paliwanag sa pamamaraang kayang mauunawaan nang lahat ng impormasyon tungkol sa pagsusuri, eksamen, gamutan, o operasyon na gagawin sa pasyente at karapatang hingin ang kanyang pahintulot bago ito maisagawa. Maipaliwanag ang mga posibleng peligro o komplikasyon.)

4. Right to Freedom of choice<\/strong>
(Karapatang mamili ng kanyang doctor sa pagamutan, humingi ng pangalawang opinion at malaman kung may alternatibong paraan ng gamutan ng inyong sakit.)

5. Right to Information<\/strong>
(Karapatang malaman ang tunay niyang sakit at lahat ng resulta ng kanyang eksaminasyon. Malaman ang lahat ng bagay tungkol sa kanyang karamdaman pati na ang mga bayaring kailangan haharapin.)

6. Right to Privacy and Confidentiality<\/strong>
(Karapatang maging pribado ang inyong impormasyon at karamdaman habang kayo ay ginagamot sa Medical Center Taguig.)

7. Right to Health Education<\/strong>

Responsibility to provide with the best of their knowledge regarding accurate and complete information about present complaints, past illnesses, hospitalizations, allergies, medications (including over-the-counter or alternative therapy medications) and other matters relating to their health;<\/strong>

2. Responsibility to comply with the treatment plan recommended by those responsible for their care;

3. Responsibility with your actions in refusing treatment or do not comply with the healthcare team\u2019s instructions;<\/strong>
(Responsibilidad sa inyong mga kilos kung kayo ay tumanggi sa paggamot o hindi pagsunod sa utos ng aming healthcare team.)
4. Responsibility in following hospital rules and regulations;<\/strong>
(Responsibilidad sa pagsunod sa mga patakaran at regulasyon ng Medical Center Taguig)
5. Responsibility in paying your hospital bills, including doctor\u2019s professional fees; This include charges once the patient\u2019s become \u201cACCOUNT ON HOLD\u201d or hospital bill reaches the maximum limit set by the management of the hospital which shall be on cash basis;<\/strong>

6. Responsibility for being considerate of the rights of other patients and hospital personnel, including doctors;<\/strong>

7. We hereby agree that all medicines, supplies and medical equipment needed by patient must only come from the hospital as part of our quality assurance;<\/strong>
