We’re making it easier for you to process your HMO approval online! Follow these simple steps to request approval without the need to visit in person.

Step 1: Email us at mctonlineapproval@gmail.com
Subject: REQUEST FOR APPROVAL
Step 2: Attach the following:
- HMO Card or Certificate of Coverage
- Valid ID (plus guardian’s ID if for a minor)
- Doctor’s request with diagnosis
- For procedures with an appointment, include the date
Step 3: Include a brief medical history explaining your reason for consultation.
When to send your request?
Send it at least 1 day before your scheduled visit or procedure.
Avoid delays or denial:
Make sure all required files are complete. Missing info may cause delays or denial of approval.
On the day of your appointment:
Bring your physical HMO card or valid ID when claiming your LOA.
Note:
Online assistance does NOT apply to OPD surgical operations or CP clearance. These need more information, and an HMO agent may have to talk directly with the patient or their representative.
