How to Request HMO Approval:

  1. Fill out the online form and attach the required documents, including your doctor’s request and a valid patient ID (or guardian’s ID for minors).
  2. Submit your request at least one day before your scheduled visit or procedure to ensure timely processing

Incomplete information or missing attachments may cause delays or denial of approval.

Bring your physical HMO card or a valid ID when claiming your LOA.This online assistance does not apply to OPD operations or CP clearance, as these require additional information. In most cases, an HMO agent may need to speak directly with the patient or their representative during approval.
